Charlotte Pooler (Clinical Scientist, Palliative/End-of-Life Care and Community Programs, AHS – Edmonton) will give an overview of Continuing Care Edmonton Zone programs and services for caregivers. She’ll provide information on how to access resources, and what clients and caregivers can expect when they are in these programs. Program areas being highlighted include Home Living, Facility Living, Supportive Living, Palliative and End of Life Care, and Transition Services (which includes CHOICE and Adult Day Programs).

This is a virtual lunch ‘n learn that is free to participants.
